Medicare Advantage vs. Original Medicare in 2026
Every fall the choice gets harder. We break down the real-world trade-offs by state and condition.
The fundamental difference
- Original Medicare (A + B) + Medigap + Part D = nationwide access, predictable cost, no prior auths.
- Medicare Advantage = bundled private plan, lower premium, network restrictions, extras like dental/vision/gym.
When Original + Medigap wins
- You travel frequently or are a snowbird.
- You have chronic conditions or expect surgery.
- You want zero prior-auth battles.
When Advantage wins
- You're healthy and want $0 premium.
- You value extras (dental, vision, OTC card, gym).
- Your area has 4–5 star plans.
The trap
You can switch from Advantage back to Original anytime, but Medigap insurers in most states can refuse you or hike premiums based on health. Only CT, ME, MA, and NY guarantee Medigap year-round.
Bottom line
Healthy and budget-focused? Try Advantage. Any chronic condition or travel needs? Original + Medigap G is worth the higher premium for the freedom.
